Sierre Discharge

Your discharge date will be discussed with your doctor on a daily basis during your hospital stay. Your discharge will be confirmed the morning before. Please make the necessary arrangements so that you can vacate your room by 10 a.m. at the latest.

If rehabilitation in the hospital is necessary, the hospital will initiate the administrative and logistical steps for the transfer. The location for rehabilitation will be determined by the hospital. The measures prescribed by the doctor and the available places will be taken into account.

Your attending doctor will receive a report on your hospital stay. If necessary, you will be asked to contact them again to continue and monitor your treatment.

Before you leave the hospital, please:

• The medical and nursing team will provide you with the necessary documents (prescriptions, medical reports, imaging examinations including a card with an access code for online viewing, confirmation of hospitalisation, blood type card, anticoagulation card, appointments, personal medication, etc.). The hospital will only dispense medication for your return home in exceptional cases.
• Do not forget to collect any valuables you may have handed in (please note the opening hours of the storage facility).
• Pay any additional costs.
• If necessary, contact the liaison nurse, who can refer you to the appropriate social medical facility. They will also assist you with the necessary steps in connection with your discharge from hospital (organisation of Spitex, family assistance service, admission to an APH, etc.).
• Make sure you have taken all your personal belongings with you.
• Return any borrowed aids (walking stick, rollator, long-handled tongs, etc.).
